Authorization for Repairs: By Submitting this Service Request form I acknowledgment of the following: 1) I authorize Simplified Yachting Services to proceed with the repair work requested; 2) I agree that Simplified Yachting Services is not responsible for a) delays caused by unavailable parts b) loss/damage to the vessel or to articles left in the vessel due to fire, theft, vandalism, or freezing; including from weather, or unforeseen events. c) Boat storage: I acknowledge that I am storing my vessel with Simplified Yachting Services at my own risk. I understand that the Company is not liable for any loss, theft, damage, or injury that may occur during the storage period, including from weather, vandalism, or unforeseen events. I agree to maintain my own insurance coverage and confirm the vessel is properly prepared for storage. I release and hold harmless the Company, its staff, and affiliates from all claims related to the storage of my vessel. 3) I grant permission for Simplified Yachting Services employees to operate the vessel for the purpose of testing and/or inspection; 4) You have insurance covering the above-referenced vessel that you will maintain throughout the time that the vessel is being repaired by Simplified Yachting Services.
